About this course

The Forklift (LF) Logbook Setup provides a structured workplace-based pathway toward completing the assessment requirements for TLILIC0003 – Licence to operate a forklift truck.

This option is suitable for participants who have access to a workplace where supervised forklift operation can occur under the guidance of a competent person.

Program Structure

  • Day 1 – Formal training covering theory and practical fundamentals
  • Demonstration of safe forklift operation including pre-start, operational and post-operational checks
  • Issue of workplace logbook and explanation of required nominal hours

Workplace Logbook Requirements

Following completion of the formal training day, participants must complete mandated nominal hours in their workplace. Logbook activities must be supervised by a person who holds a current High Risk Work Licence – LF.

The supervising licence holder is responsible for overseeing safe operation and signing off completed logbook hours.

Final Assessment

Once the required nominal hours (as determined by your assessor) have been completed and documented, participants may book into a formal assessment session.

The assessment day is conducted as the third day of a standard 3-day forklift course and includes both theory and practical components.

Licensing Requirement

If you are required to operate a forklift truck, you must hold a current High Risk Work Licence – LF.

Pre-requisite

Pre-Requisite

There are no formal training pre-requisites for this program. However, participants must meet the following entry requirements:

  • Be at least 17 years and 6 months of age at commencement of training
  • Be at least 18 years of age at the time of final assessment (as required for High Risk Work Licence assessment)
  • Possess basic English language, literacy and numeracy skills, including the ability to read, write and perform basic calculations relevant to forklift operation

Enrolment Requirements

  • Be at least 18 years of age at the time of training and assessment
  • Provide 100 points of identification
    (WorkSafe QLD ID requirements)
  • Hold a valid USI (Unique Student Identifier)
    (Obtain a USI here)
  • Be able to read, write and communicate in plain English to a level sufficient to safely participate in training and assessment

Requirements on the Day of Training & Assessment

  • Original 100 points of identification (as per WorkSafe QLD requirements)
  • Steel-capped or composite safety boots
  • Long-sleeved high-visibility shirt (high-visibility vests available for loan)
  • Long work pants
  • Hard hat (limited supply available for loan)

Momentum High Risk Training provides all required training materials and stationery.

  • Participants may bring their own lunch (fridges and crib facilities are available); or
  • Multiple food outlets are located nearby at Mackay Harbour

Course outcomes

Final Assessment & Certification

Once you have completed the required logbook hours and your documentation has been signed off by your supervising licence holder and assessor, you may book into a formal assessment session.

Upon successful completion and being deemed competent, you will receive a
Statement of Attainment for TLILIC0003 – Licence to operate a forklift truck
issued by Momentum High Risk Training (RTO 46410).

A Candidate Assessment Summary (CAS) will also be issued via email. This document is required when applying for your High Risk Work Licence (HRWL).

To apply for your High Risk Work Licence (LF class), you must contact
WorkSafe QLD
within 60 days of completing your assessment.

Please retain your Candidate Assessment Summary (CAS) as evidence of assessment until your HRWL card is issued.
